  • NEW OFFICIALS: Fill out the form in order to receive your MASO Badge after you have successfully paid your Membership Fee via MASO RQ+. You badge fee is included in the price of your Membership Payment.

  • EXISTING OFFICIALS: Fill out the below form if you have lost/misplaced your MASO Badge. Please note that there is a $10.00 replacement badge fee that will need to be sent after submission of this form (you will receive information via email on where to send the Venmo payment.
