CURRENT MASO OFFICIALS: If a replacement badge is needed (lost/misplaced/damaged), you will be required to pay a $10 badge replacement fee (you will receive a Venmo payment request via your username you provide in the below form). Once payment is sent, your replacement badge will be mailed out. If you do not have Venmo, please provide how you would like to submit payment via the last field.
NOTE: Do not fill out this form to request a MASO Badge - this is automatically mailed out once your Membership Registration is submitted via RQ+ Registration.
